Congratulations to Pon Sang Chan, MD, who has been recognized for seeing more than 90% of his patients at least one time in the last 12 months and providing outstanding clinical care for their patients.
Access to comprehensive, quality health care services is important for promoting and maintaining health,
preventing and managing illness, reducing unnecessary disability and premature death, and achieving health equity.
Access to good health care is vital even for patients without risk factors because it promotes healthy behaviors, such as healthy eating and exercising. Those at risk who have access to good health care benefit from protection, such as vaccinations. They also benefit from screenings that identify conditions, such as hypertension and diabetes, before they progress. In short, access to health care impacts everyone’s quality of life.
Having a primary care provider (PCP) who serves as the usual source of care is as imperative. PCPs can develop meaningful and sustained relationships with patients and provide integrated services. Studies show that having a regular PCP is associated with higher patient trust in the providers, and the increased likelihood that patients will receive appropriate care.
We appreciate Dr. Chan for his commitment and dedication to high-quality patient care and would like to recognize for this achievement.
Throughout 2018, a group of physicians received MDX Hawai`i Excellence in Kupuna Care awards based on select components of high-quality primary care, including metrics which reward preventative care, care coordination and convenient access to care.
